Cytostatika

Cellegift, også kalt kjemoterapi eller cytostatika, er medisiner som brukes for å behandle selve kreftsykdommen. Cellegift ødelegger kreftcellene. Også friske celler skades, derfor opplever mange ulike bivirkninger av cellegift.

Målet med cellegiftbehandlingen

  • Kurativ behandling har som mål å helbrede sykdommen
  • Livsforlengende behandling har som mål å forlenge livet
  • Symptomlindrende behandling har som mål å lindre plagsomme symptomer

Hvor lenge og hvor mye vi skal behandle selve kreftsykdommen, er ofte en vurdering. Dine ønsker og kunnskap om sykdommen er vesentlig å ta hensyn til. Allmenntilstanden din har stor betydning for hvilken behandling du bør få. Noen ganger må vi starte behandlingen for å se hvordan den virker på deg og kreftsykdommen.

Informasjon fra deg og dine pårørende, samt dialog mellom deg og legen, er avgjørende når vi skal planlegge, starte og avslutte den videre behandlingen. Du får informasjon om hva som er hensikten med behandlingen som blir tilbudt.

Før

Før kuren startar må du ta blodprøver. Dette gjer du oftast same dag eller dagen før. Ved enkelte typar cellegiftkurar skal det også takast EKG før første kur.

Vi anbefaler at du et eit lett måltid før cellegiftbehandlinga. 

Under

Vanlegvis blir cellegiftkurar gitt intravenøst, det vil seie gjennom ei blodåre. Ved utvalde kreftsjukdommar kan cellegift også settast direkte i urinblæra eller i ryggmergsholrommet. Cellegift som tablettar eller kapslar skal svelgast heile med rikeleg mengde drikke. Varigheita av éi cellegiftbehandling kan variere frå nokre minutt til fleire dagar. Intervallet mellom kurane er vanlegvis éi til fire veker.

Ein cellegiftkur kan bestå av ulike typar cellegifter som blir gitt fortløpande etter kvarandre. Nokre kurar krev innlegging på sengepost, men oftast kan kurane givast på poliklinikkar.

Etter

I samband med cellegiftbehandling er det viktig med god hygiene fordi nedbrytningsprodukt blir skilde ut gjennom slimhinner, urin, avføring og oppkast. Dette gjeld særleg dei tre første døgna etter avslutta kur. Her følger nokre generelle råd:

  • Vask hendene godt med varmt vatn og såpe etter toalettbesøk

  • Bruk papir for å tørke opp eventuelt søl, vask deretter med vatn og såpe

  • Kast bind og bleier i ein eigen plastpose

  • Vask klede og sengeklede som blir tilsølt, med éin gong

  • Ved seksuell kontakt bør ein bruke kondom

Ved å følge desse forholdsreglane, kan du trygt vere saman med andre akkurat som før – også barn og gravide. Du bør likevel unngå nærkontakt med personar som har symptom på infeksjonssjukdom.

 

Behandling med cellegift kan gi dårleg matlyst. Dersom du har vedvarande nedsett matlyst over fleire dagar er det viktig at du kontaktar sjukepleiaren din eller ein annan kontaktperson på sjukehuset. Det er viktig at du drikk godt, minimum to liter væske dagleg. Dette fordi mange cellegifttypar og deira nedbrytingsprodukt blir skylde ut med urinen. Rikeleg inntak av væske kan òg førebygge kvalme.

Energirike drikker og vatn er bra. Når det gjeld mat, er det ofte lettare å få i seg kald enn varm mat, salt mat i staden for søt mat og flytande i staden for fast. For eksempel kan salat og brød, eggerøre og røykalaks eller tomatsuppe smake godt.

Det kan også vere lurt å spe på med næringsrike mellommåltid, som ost, tørka frukt, nøtter og næringsrike drikker. Det finst ulike typar næringsdrikker som du kan få på resept eller kjøpe på apoteket. Du kan også tilsette næringspulver i matlaginga. Sjå også eigne brosjyrar for meir informasjon om ernæring. Prøv å lage appetittvekkande og innbydande måltid, men ver oppmerksam på at cellegiftbehandling ofte fører til at maten smakar annleis enn før.

Cellegiftbehandling er knytt til fleire biverknader fordi behandlinga også påverkar dei normale cellene i kroppen. Intensiteten og varigheita av biverknadene avheng av type cellegiftkur og varierer frå person til person. Det er viktig at du nøye følger råd frå sjukepleiar og lege om når du skal ta kontakt med helsepersonell etter kuren. Eksempel kan vere feber eller kraftig diaré. Ved neste kur eller kontroll er det viktig at du er open om plagene du eventuelt har hatt sidan førre kur.

Dei fleste cellegiftkurar set ned beinmergsproduksjonen av raude og kvite blodlekamar og dessutan blodplater. Talet på kvite blodlekamar søkk etter behandlinga. Dette oppstår oftast 5 til 15 dagar etter kur og avheng av kva kur som er gitt. I denne fasen er du ekstra utsett for infeksjonar. Unngå derfor nærkontakt med menneske som openbert har symptom og teikn på infeksjon.

Cellegiftbehandling kan redusere nivået av blodplater, dette kan medføre auka blødingsfare. Etter lengre tids behandling kan også produksjonen av raude blodlekamar settast ned og det kan bli aktuelt med blodoverføring.

Alle biverknadene som påverkar beinmergen er forbigåande og det er ingen praktiske ting du kan gjere for å betre denne tilstanden. I utvalde tilfelle kan det bli aktuelt å gi medisinar som stimulerer produksjonen av kvite blodlekamar.

Fleire cellegiftkurar kan gi kvalme og brekkingar. Ubehaget kjem ofte nokre timar etter avslutta behandling og kan halde fram i 1–4 døgn etter siste behandlingsdag. Dei siste åra har vi fått svært effektive kvalmestillande medikament mot kvalme som er forårsaka av cellegift. Det er derfor svært viktig at du tar slike medikament som anbefalt av legen din på sjukehuset. Psykiske faktorar som stress og angst kan også påverke opplevinga av kvalme, berre tanken på neste kur kan vere nok til å utløyse brekkingar hos enkelte. 

Slappheit er ein vanleg biverknad av cellegiftbehandling dei første 1–3 døgna etter behandling. Deretter vil slappheita gradvis minke over dei neste dagane.

Nokre cellegiftkurar kan medføre varierande grad av håravfall på hovudet og at kroppshår elles blir fortynna. Dette skjer vanlegvis etter 1–2 kurar. Håret vil komme gradvis tilbake etter avslutta behandling. NAV gir økonomisk støtte til parykk og hovudplagg når pasienten har attest frå lege.

Nokre få cellegifter kan gi kortvarige smerter i armen fordi det oppstår ein irritasjon av blodåresveggen. Andre cellegifter kan gi skade i normalt bindevev og muskulatur dersom cellegift kjem utanfor blodåra.

Somme cellegiftkurar kan gi feber nokre timar etter behandling. Dette er ein normal reaksjon, og er ikkje farleg. Det er likevel svært viktig å vere merksam på feber som oppstår mellom kurane, da dette kan vere det første teiknet på ein livstruande infeksjon. Ved feber over 38 grader mellom kurane må lege kontaktast med ein gong.

Fleire cellegifter kan forårsake slimhinnebetennelse i munn og svelg, tarmar, magesekk, urinblære og underlivet hos kvinner. Slimhinnebetennelse i munnslimhinna kan forårsake smerter ved tygging og svelging av mat og dermed nedsett matlyst. Det kan også førekomme sår i magesekk og i slimhinnene til tynntarmen.

Sårheit i blæreslimhinna kan medføre svie ved vasslating, og dessutan hyppig vasslating, og sårheit i skjeden kan forårsake smerter ved samleie. Det er vanleg at såra lækjer seg sjølve etter nokre dagar og i god tid før etterfølgande cellegiftbehandling.

Under behandling kan kvinner få uregelmessig menstruasjon, eller ho kan stanse heilt. Likevel bør kvinner bruke prevensjon både i behandlingsperioden og i cirka eitt år etter behandlinga er avslutta, på grunn av risiko for skadar på eggcellene under behandlinga.

Produksjon av sædceller hos menn blir påverka og dette gjer at befruktningsevna kan bli sett ned. Menn bør også bruke prevensjon for å hindre overføring av skadde sædceller til partnaren. Grada og varigheita av nedsett befruktningsevne er først og fremst avhengig av alder: Jo yngre, desto betre er sjansen for å bevare befruktningsevna.

Det kan oppstå forbigåande skade av små nerveforgreiningar som kan gi kjensleløyse, nomenheit og kuldesanseinntrykk på tær og fingertuppar. Dersom slike biverknader oppstår, forsvinn dei vanlegvis nokre veker eller månader etter avslutta behandling. I nokre tilfelle vil ein måtte stoppe eller endre på cellegiftbehandlinga for å unngå varige plager.

Vær oppmerksom

Når du er under cellegiftbehandling er du ekstra utsett for infeksjonar. Unngå derfor nærkontakt med menneske som openbert har symptom og teikn på infeksjon.

Ved feber over 38 grader mellom cellegiftkurane må du kontakte lege med ein gong.

    If you are coming to Breivika from Tromsø city centre, the easiest option is to take bus number 20 or 21 from Fredrik Langes gate. This bus journey takes approximately 15 minutes.

    When you enter through the main entrance from the bus stop, you will be in wing A, level 6. The reception is located to your right.

    Are you coming from Narvik?

    The Health Express runs from Narvik to Tromsø. Individuals who have been referred for travel due to health reasons can use the Health Express.

    ​​Kulturseksjonen i UNN arrangerer jevnlig lunsjkonserter på Pingvinen Scene ved hovedinngangen. Konsertene starter som regel klokka 1130. Informasjon om hvem som spiller henger i heisene. 

    Musikk er også helse, og med jevne mellomrom arrangeres det ”musikkvandring” i sykehuset. Ta kontakt med sykepleier på avdelinga dersom du er innlagt hos oss og ønsker besøk av musikerne våre.

    Visitors can use all bicycle parking around the hospital. If you want parking near the main entrance, it is most practical to use:

    The entrance to the Penguin Hotel at UNN Tromsø is through the hospital's main entrance at A6. You can check in at the hotel reception upon arrival. There is always someone available to welcome you. The hotel is open 24 hours a day.

    The maternity hotel is part of the Penguin Hotel. The rooms are equipped with changing tables. The maternity hotel has midwives available for added security for the new family.

    There is a TV and internet access in all hotel rooms.

    Breakfast is included in the room rate. Lunch, dinner, and evening meals are available for purchase in the hotel restaurant according to the current opening hours for those who do not have all meals included.

    Vardesenteret

    Vardesenteret er et samarbeid mellom UNN og Kreftforeningen, og er en møteplass for kreftsyke, pårørende og etterlatte. Her treffer du ansatte fra Kreftforeningen og UNN, samt frivillige og likemenn. 

    Vardesenteret er tilknyttet UNN i Breivika og befinner seg i E-fløya. For å komme dit må du først til plan 2, fløy D. Følg skiltene derfra. Er du usikker kan du spørre en frivillig sykehustvert som er kledd i rød vest.

    If you are coming to Breivika from Tromsø city centre, the easiest option is to take bus number 20 or 21 from Fredrik Langes gate. This bus journey takes approximately 15 minutes.

    Are you coming from Narvik?

    The Health Express runs from Narvik to Tromsø. Individuals who have been referred for travel due to health reasons can use the Health Express.

    The hospital in Harstad is located at St Olavs gate 70, directly opposite the town centre.

    On foot

    If you choose to walk, you can reach the hospital in about 15 minutes. Please note that Harstadgårdsveien can feel both long and steep if you have difficulty walking.

    Bus and express boat

    If you are coming to the hospital from Harstad town centre (from Harstad city terminal), the easiest options are:

    • Bus number 11 towards Samamoa (if you take route 11 towards Gansås, you will need to walk up Gamle kirkevei, which is steep).
    • Bus number 15 towards Fagerlia

    Both bus routes depart from the closest busstop  (turn right when coming from the Harstad express boat quay).

Patients can help themselves to food in a new staffed café, and they receive good assistance from a host if they are unable to bring the food to the table.

    The hospital in Harstad is located at St Olavs gate 70, directly opposite the town centre.

    On foot

    If you choose to walk, you can reach the hospital in about 15 minutes. Please note that Harstadgårdsveien can feel both long and steep if you have difficulty walking.

    Bus and express boat

    If you are coming to the hospital from Harstad town centre (from Harstad city terminal), the easiest options are:

    • Bus number 11 towards Samamoa (if you take route 11 towards Gansås, you will need to walk up Gamle kirkevei, which is steep).
    • Bus number 15 towards Fagerlia

    Both bus routes depart from the closest busstop  (turn right when coming from the Harstad express boat quay).

    It is recommended to use the journey planner from Troms County Traffic (Svipper). The information in the journey planner is always up to date.

    Transport

    The hospital is located at Stasjonsveien 61, approximately three kilometres north of the centre. It takes about 25 minutes to walk from the centre.

    If you arrive in Narvik by the airport bus from Evenes, you can either walk or take a taxi to the hospital. The Health Express has a start and stop at the hospital on Furumoen. Remember that travel expenses must be agreed upon in advance with Patient Travel.

    The tunnel from the E6 (Furumotunellen) is the main access road to the hospital.

    There are established footpaths through Narvik Cemetery and up to the hospital, in addition to pavements along Stasjonsveien.

    Public Transport

    Several new bus services have been established to the new hospital on Furumoen.

    It is recommended to use the journey planner and search for routes to/from UNN Narvik. The information in the journey planner is always up to date.

    Patients and relatives who rely on a car to Furumoen can park in the public parking area outside the main entrance. Additionally, there are some public parking spaces outside the main entrance to Furumoen nursing home and outside the emergency reception/urgent care.

    Due to high demand, it is encouraged to use the bus to and from Furumoen. Good bus connections have been established by Reis Nordland; check the route planner.

    There are disabled parking spaces at the main entrance (maximum 4 hours), in the main parking area, at Furumoen nursing home, and at the emergency reception/urgent care.
    Remember to display the blue parking permit clearly in the front window.
